Your First Visit
Your first visit to Stephens Dentistry establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, such as your medical history. We also give you time to get to know your doctor. What to expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling.
Our office is a completely digital office. This means that when you come in for your appointment, you will fill out the registration and medical information on a patient laptop. Your information will then be conveniently linked to your record for the doctors to review. To ensure that you get the most out of your appointment, we recommend that you come 10 minutes early to fill this information out.
We ask that you bring a photo ID as well as any necessary dental insurance information with you to your first appointment. We will make copies of these to keep your record complete. If at any time your insurance changes, please be sure to let us know.
At your first visit you will be able to meet the doctor and the hygienist. We will take a complete set of x-rays to ensure your dental record is as accurate as possible. If you are transferring from a different dentist and have current x-rays, you are welcome to have them transferred to our office. We accept digital copies e-mailed to us at office@stephensdentistry.com or hard copies can be mailed to our office. The doctor will also complete an initial comprehensive exam to get a complete look at your oral health. Dr. Robert or Dr. will be happy to address any concerns you may have at this time as well. The hygienist will perform your prophylaxis (cleaning) and go over your oral hygiene routine with you. We also recommend a fluoride treatment and an oral cancer screening which your hygienist will be happy to review with you.
Cancellations
We realize that your time is important, and so is ours. We require 48 hours notice if you need to reschedule or cancel your appointment. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment or reschedule without sufficient notice.
Directions and Parking
Our office is located on the 9th floor of the Rotary building in Downtown Evanston. Our building offers handicapped parking at no charge within the building garage. We are also conveniently located close to lots of parking and just a block away from the ‘L’. We are walking distance from the Davis Street and Dempster Street Purple Line stops. There is street parking in front of the building, as well as several parking garages located within a block of us — one on Davis Street and one on Sherman under the Best Western.
